Double Gram-Negative Coverage

Double Gram-Negative Coverage

Conditions

Add double gram-negative coverage if:

  • Hemodynamic instability requiring pressure support
  • History of infection or colonization with an ESBL-producing pathogen and/or multi-drug resistant Pseudomonas

Antibiotic Treatment

OR

Tobramycin IV (per pharmacy)

Use if:

  • Patient is on fluoroquinolone prophylaxis
  • Has received acute fluoroquinolone treatment w/in past 30d, or
  • Relevant cultures in past 6 months reveal a fluoroquinolone-resistant Gram-negative pathogen
