In the setting of rapidly improving or worsening renal function, calculations of CrCl are inaccurate. Dosing should be based on estimated renal function instead.

If improving renal function, actual CrCl is likely higher than calculated CrCl.

If worsening renal function, actual CrCl is likely lower than calculated CrCl.

Not On Dialysis

CrCl > 50

Usual adult dose

CrCl 0 - 10
  • Meningitis or endocarditis: 2g IV q12h
  • Pyelonephritis: 2g IV q24h
  • Uncomplicated UTI: 1 g IV q24h
CrCl 10 - 29
  • Meningitis or endocarditis: 2g IV q8h
  • Pyelonephritis: 2g IV q12h
  • Uncomplicated UTI: 1 g IV q12h
CrCl 30 - 50
  • Meningitis or endocarditis: 2g IV q6h
  • Pyelonephritis: 2g IV q8h
  • Uncomplicated UTI: 1 g IV q8h

Intermittent Dialysis

  • Meningitis or endocarditis: 2g IV q12h
  • Pyelonephritis: 2g IV q24h
  • Uncomplicated UTI: 1 g IV q24h

Give doses after HD on HD days, if possible

Continuous Dialysis

Load 2g then 1-2g IV q6h
