Dosing

Severe infection (febrile neutropenia, PNA, CNS, Bone/joint): 2 g IV q8h

Mild-moderate infection: 2 g q12h

Cystitis: 1 g q12h

CrCl 0 - 10CrCl 10 - 30CrCl 30 - 60CrCl > 60- Severe infection: 1 g q24h

  • Mild-moderate infection: 500 mg q24h

  • Cystitis: 500 mg q24h- Severe infection: 2 g q24h

  • Mild-moderate infection: 1 g q24h

  • Cystitis: 1 g q24h- Severe infection: 2 g q 12h

  • Mild-moderate infection: 2g q24h

  • Cystitis: 1 g q24hUsual adult dose

General Information

Broad spectrum antimicrobial with gram positive and gram negative coverage including Pseudomonas.

Used especially nosocomial infections involving resistant organisms.

  • Altered mental status, confusion

  • Allergy (immediate & delayed)

  • Positive Coombs

  • Cytopenias

  • Non-convulsive status epilepticus
