Dosing

  • For endocarditis, bacteremias, or other deep-seated infections: 2 gm IV q4h

  • For most other infections: 2 gm IV q6h

Mostly used for MSSA infections Drug of choice for CNS MSSA infection

  • Allergy/rash (immediate or delayed)

  • Seizures

  • Cytopenias

  • Eosinophilia

  • Interstitial nephritis

  • Abnormal liver enzymes

  • Serum sickness

May increase serum methotrexate levels May DECREASE INR in patients receiving warfarin

Can cause phlebitis if given peripherally. Central line preferred for tolerance
