General Information

Gram negative bacilli, of the Enterobacteriaceae family

Normal GI flora.

Most predominant Gram negative bacillus human pathogen.

Most common cause of Urinary Tract Infection.

Also common:

  • Intra-abdominal infection/abscess
  • Nosocomial pneumonia
  • Bacteremia
  • Prosthetic device infection
  • Gastroenteritis, ranging from travelers diarrhea to invasive infection with hemolytic-uremic syndrome
  • Myriad other manifestations
  • Note endocarditis is rare

Treatment of choice for ESBL strains is a carbapenem.

Routine Practices; however, consider contact precautions for incontinent adults if stool cannot be contained or for adults with poor hygiene who contaminate their environment. 

Contact precautions apply to children who are incontinent or unable to comply with hygiene.
