Dosing

General Information

Broad spectrum antimicrobial with Gram positive and Gram negative coverage including Pseudomonas.

Used especially for nosocomial infections involving resistant organisms.

Monitor renal function, hepatic function, and CBC periodically.

  • Phlebitis at infusion sites
  • Diarrhea
  • Allergic reactions
  • Cytopenias
  • Neurotoxicity (especially high doses in setting of renal failure): convulsions, encephalopathy, confusion, disorientation, hallucinations
  • LFT elevations

Probenecid: increased cephalosporin concentration.

Cefepime dose optimization policy followed due to time dependent pharmacokinetics and pharmacodynamics. Activity is correlated to keeping free drug concentration above the MIC for an adequate time during the dosing interval.

Distribution: wide distribution including blister fluid, bronchial mucosa, peritoneal fluid, prostate, bone, CNS penetration with inflamed meninges (must use high dose).
