Step 1

Patient has 2 or more SIRS criteria, organ dysfunction, AND a confirmed/suspected source of infection

Step 2

Initiate the Fever/Sepsis Care Set (p.8)

Blood Cultures x2, Lactic Acid, CBC and Plt w Diff, CMP, etc.

Step 3

Prompt initiation of Antibiotics

  • Piperacillin-tazobactam*
  • Ceftriaxone
  • Meropenem
  • Cefepime
  • Levofloxacin
  • Ciprofloxacin
  • Ampicillin-sulbactam
  • Cefazolin
  • Eravacycline
  • Ceftazidime-avibactam
  • Imipenem
  • Vancomycin
  • Linezolid
  • Gentamicin
  • Tobramycin
  • Ceftolozane-tazobactam
  • Cefiderocol
  • Azithromycin
  • Doxycycline
  • Metronidazole
  • If piperacillin-tazobactam ordered, contact pharmacy for first dose to be given over 30 min and then start extended infusion protocol after.

Step 4

Repeat lactic acid if initial >2. 30ml/kg fluid bolus for patients with hypotension, "soft" pressures, or lactic acid >4.

Use Cheetah for ALL patients who qualify for bolus and document under "Hemodynamics"
