Splenectomy Vaccination Guideline


  • 1 dose of PCV13 followed by 1 dose of PPSV23 ≥8 weeks later.
  • If primary series already received, revaccinate with PPSV23 every 5 years.
  • 1 dose recommended for all adults who have not been previously vaccinated or if vaccination status is unknown.
  • No revaccination required.
  • 2 doses at least 8 weeks apart.
  • If primary series already received, continue revaccinating every 5 years.
  • 2 doses of MenB-4C at least 1 month apart, if not already received. or
  • 3 doses of MenB-FHbp at 0, 1 to 2, and 6 months, if not already received.

Revaccination recommended 1 year after completing the primary series, and then every 2 to 3 years thereafter.

  • 1 dose annually at the start of influenza season.


Patients should receive all routinely recommended age-appropriate vaccinations as recommended by ACIP.

Asplenia alone is not a contraindication for any vaccination, including live vaccines.

Immunocompromising conditions such as hematologic malignancies and HIV infection can co-occur with asplenia and can be contraindications to live vaccinations.
