Empiric Treatment

Management

Obtain 2 blood cultures and/or expectorated sputum gram stain and culture if severe inpatient CAP or being started on empiric MRSA and Pseudomonas aeruginosa coverage

Pneumonia Severity Index (PSI) and CURB-65 scores to assess disease severity

Clinical Considerations

No healthcare exposure

Pulmonary Symptoms

  • Cough
  • Dyspnea
  • Tachypnea
  • Crackles or rales
  • Hypoxemia
  • Sputum production

Systemic Symptoms

  • Fever
  • Leukocytosis
  • Chills
  • Fatigue
  • Tachycardia

Chest X-Ray

  • Single or multi-lobe involvement with consolidations, opacities, and/or interstitial infiltrates
