Dosing

General Information

Administration

  • Infuse over 1 hour
  • Not to be infused by rapid or bolus
  • PO dosing: administer with or without food and separate from antacids and multivitamins by 4 hours (before) or 8 hours (after)

Diluent

  • IV solution in ready-to-use 250 mL flexibags in 0.8% sodium chloride is yellow
  • Do not mix or infuse simultaneously with other drugs or additives
  • Pneumonia including CAP, HAP, aspiration
  • Some intra-abdominal infections & SSTIs
  • CNS infections

Tier 4 Protected Antimicrobial

  • Non-Formulary Antimicrobial: Requires approval from Antimicrobial Stewardship Subcommittee Chairpersons, regardless of inpatient or outpatient status. These medications are not stocked in the Pharmacy and require 24-48 hours for approval and procurement of the drug.
  • Monitor QTc by EKG in patients with increased risk (i.e. cirrhosis)
  • WBC and other signs of infection
  • Glucose dysregulation
  • QTc prolongation
  • Dysglycemia
  • Rash
  • Tendinopathy and rupture
  • GI upset
  • Weakness exacerbation in myasthenia gravis
  • CNS toxicity including confusion, psychosis
  • QTc prolongation - Increased risk with other agents that prolong QTc
  • Divalent cations including Ca, Mg - Decreased absorption
  • Warfarin - Increased INR
  • Rifampin - Decrease moxifloxacin levels
