Dosing

General Information

Administration

  • Infuse over 1 hour
  • May reduce infusion rate for infusion-related reactions
  • Flush line with NS prior to administration
  • Protect from light (bag, not the drip chamber or tubing)

Preparation

  • Aseptically reconstitute vial with 5mL of preservative-free NS or D5W to each 50 or 100mg vial
  • Swirl gently to dissolve, do not shake (foaming)
  • Further dilute 50-150mg in 100mL of NS or D5W

Diluent

  • 100 mL NS or D5W

Therapy of suspected and confirmed invasive fungal infections

Tier 3 Protected Antimicrobial

  • Limited to use for the listed indications. For all other indications, the antimicrobial must be ordered by an ID physician or an ID consult placed. Prospective audit with feedback with provided by ASP/Pharmacy and utilization/compliance reported quarterly.

AHS Use Criteria:

  • Empiric therapy for suspected or proven invasive candidiasis in critically-ill patients
  • Treatment for candidiasis in patients with documented or suspected azole resistance
  • Invasive candidiasis refractory to treatment with azoles
  • Invasive fungal infection where azole antifungals are contraindicated due to drug interactions/allergy/adverse reaction
  • Treatment for invasive aspergillosis infection
  • Periodically monitor liver and renal function, CBC
  • Monitor for infusion reactions
  • Increased liver enzymes
  • GI upset
  • Phlebitis
  • Anemia

Increases levels of sirolimus and nifedipine (mild)
