Dosing

General Information

PO only

  • Mix with 3-4 ounces of cool water before ingestion

  • With or without food

  • Urinary Tract Infection, especially involving ESBL producing organisms

  • Offlabel - Prostatitis

Tier 3 Protected Antimicrobial

  • Limited to use for the listed indications. For all other indications, the antimicrobial must be ordered by an ID physician or an ID consult placed. Prospective audit with feedback with provided by ASP/Pharmacy and utilization/compliance reported quarterly.

AHS Use Criteria:

  • Treatment of complicated and uncomplicated cystitis in patients where fosfomycin is the only available oral option, based on patient allergies or susceptibility data

  • Susceptibility to fosfomycin must be ordered/available when treating an MDRO or off-label organism, or when used in pregnancy

  • Not be used in patients with renal impairment due to decreased urine concentration

Resolution of UTI symptoms

  • GI upset, especially diarrhea

  • Abdominal pain

  • Headache

  • Eosinophilia, neutropenia

  • Hypokalemia

No significant interactions
