Dosing

Standard dose: 600 mg IV q12h

High dose: 600 mg IV q8h (may consider in morbidly obese patients (>150kg) or >100kg when severely ill (i.e. in ICU))

Note: No adjustment needed for renal impairment

General Information

Targeted or empiric therapy for gram positive infections including skin and soft tissue, and pneumonia.

Covers MRSA and VRE.

CBC at least once/week

Visual testing for therapy >3 months or if symptoms develop on therapy.

  • Myelosuppression (usually with >2weeks therapy)- reversible and dose related

  • Serotonin syndrome

  • Rash

  • Elevated liver enzymes

  • Lactic acidosis

  • Peripheral/optic neuropathy with prolonged courses

SSRI and other serotonergics/MAOIs - Increased risk of serotonin syndrome.

Rifampin decreases linezolid levels.
