Spectrum of Activity

Dosing

General Information

In most instances, fosfomycin IV should only be used as part of a combination regimen.

Fosfomycin IV should ONLY be used when it is considered inappropriate to use antibacterials that are commonly used for initial treatment of the infections listed below.

  • Osteomyelitis
  • Complicated urinary tract infections
  • Nosocomial lower respiratory tract infections
  • Bacterial meningitis
  • Bacteremia that occurs in association with, or is  suspected to be associated with, any of the infections listed above
  • Plasma electrolytes (in particular sodium, potassium and phosphate) and fluid balance must be monitored regularly during therapy, in particular when using the 
    high dose regimen (>16 g/day in adults)
  • GI upset
  • Erythematous eruption
  • Injection site reactions
  • Hypokalemia
  • Hpernatremia

Antimicrobial class: Phosphonic acid derivative

Average serum half-life: 2 hours; Elderly and/or critically ill patients: 3.6 to 3.8 hours

  • 1 g fosfomycin IV contains 14 mmol (320 mg) sodium, 
    equivalent to 16% of the WHO recommended maximum daily dietary intake of 2 g sodium for an adult.
