General Information

  • Weekly CBC with differential, renal function, and chemistries.
  • Baseline and weekly creatine kinase (discontinue if symptomatic and CK > 1000 U/L (5×ULN) or asymptomatic and CK > 2000U/L (10×ULN)
  • Myopathy including rhabdomyolysis
  • Nausea, vomiting, diarrhea
  • Rash
  • Phlebitis
  • Increased INR

Please refer to Lexicomp for more information

Please refer to Lexicomp for more information

Inactivated by pulmonary surfactant and therefore should NOT be used to treat pulmonary infections

Antimicrobial class: Cyclic lipopeptide, depolarizes bacterial cell membrane

Average serum half life:

  • 1-6 years old: 5 hours
  • 7-11 years old: 6.25 hours
  • 12-17 years: 7.3 hours
  • Adults: ~ 8 hours
  • Due to quicker elimination in pediatric patients it is difficult to obtain similar AUC (i.e. drug exposures) and thus difficult to obtain bactericidal activity in those with normal renal function

Route of Elimination: 78% of the dose excreted in urine as unchanged drug; feces (6%)
