Sepsis with NO FOCUS Age > 60 days

Clinical/Diagnostic Considerations

  • No pulmonary, central nervous system, osteoarticular, urinary tract or abdominal focus identified on history, physical or laboratory investigations.
  • If prominent diffuse erythema (especially adolescent females) consider Toxic Shock Syndrome (TSS)
  • Adequate volume blood cultures, urine analysis AND microscopy, urine culture, chest radiograph, throat swab, viral (esp. influenza) swabs, consider abdominal imaging.

Suggested blood culture volumes:

  • 1 -10 years- 2- 10mL (generally 1 ml + age)
  • 10-15  years - 10-20 mL (use adult blood culture bottles)
  • ≥ 15 years -  20-40 mL (use adult blood culture bottles)

Empiric Antimicrobials

If suspicion for Staphylococcus aureus (or TSS) consider addition of:

(MRSA)

Usual Duration

Reassess antimicrobials at 36 hours of blood culture incubation and clinical evolution
