General Information

Prior Severe Cutaneous Adverse Reaction (SCAR) to cephalexin

  • Allergy

  • Non-allergic rash

  • Neutropenia, thrombocytopenia

Cellulitis, osteomyelitis, UTI with susceptible pathogens, sinusitis, CAP and AOM. Has in vitro activity against methicillin susceptible S. aureus, streptococci, E. coli (if susceptible), Klebsiella spp. (if susceptible) and Proteus mirabilis (if susceptible) . Also has activity against Peptostreptococci. 

Can be used as empiric therapy for patients with cystitis alone or with one dose of IV aminoglycoside (tobramycin or gentamicin) for initial treatment of pyelonephritis in patients who will be discharged from ED and will be followed for culture results and clinical course.

Once the urine culture isolate is reported as susceptible (S) to both cefazolin and cephalexin then either cefazolin or cephalexin is considered appropriate for treating either a cystitis or a febrile UTI /uncomplicated pyelonephritis.
